Filling in the form

Read the form field by field: names, dates, reasons.

Key words

Read these first: you will meet most of them in the document.

  • il modulothe form

Deve compilare questo modulo e firmarlo in fondo.

Masculine. The paper or online form you fill in at every office. Un modulo is also a module, as in a course.

  • compilareto fill in (a form)

    Ho compilato la domanda online.

A false friend: compilare un modulo is to fill in a form, not to compile it. La domanda here is the application, not a question.

  • il nomethe name / first name

    Qual è il tuo nome?

Masculine despite the -e. On forms nome is the first name and cognome the surname. In speech Italians ask come ti chiami? far more often.

  • l'indirizzothe address

    Mi scrivi il tuo indirizzo?

Masculine, spelled with one d and a double z — the opposite of English. In an Italian address the number comes after the street: via Garibaldi 15.

  • il codice fiscalethe tax code

Per aprire un conto ti serve il codice fiscale.

Masculine. A sixteen-character code built from your name, date and place of birth, issued by the Agenzia delle Entrate. You need it for a lease, a phone contract, a job and a doctor.

  • rinnovareto renew

Devo rinnovare il passaporto prima di partire.

Regular -are. The noun is il rinnovo: il rinnovo del permesso di soggiorno. It also means to refurbish: rinnovare la cucina.

  • la marca da bollothe revenue stamp

Per la domanda serve una marca da bollo da sedici euro.

Feminine. A stamp bought at a tabaccheria and stuck on many official applications to pay a fixed fee. Offices often ask for one, so check before you go.
